You have to do risk analysis for this new requirement.Check that new requirement is added in SRS.Then add test cases relating to new requirement.Also, verify that adding this requirement has any effect on existing functionality .

You must make sure that the defect is already logged or not. If it was logged, what is the status of the defect ? If it was logged & closed then what are the comments made against defect. Then you can make a decision on that defect.

Traceability matrix is where the test plan author,generally the TL,reviews the testcases prepared by test engineers.Here the TL verifies whether the testcases written cover all the Business Requirements or not.He also rejects/deletes extra/not required testcases.

Build version Id is the id given to the application when it is released with in the company during different life cycles and Software verion Id is the id given to the appliction when it is released outside the company for the customer
