Keep it simple and to the point. You are generally reporting to non-technical people so use a summary report based on the following, and use plain language.

Summary - This is a management so summarise what was tested and what happened. Point to any relevant documents here.
Variances - If any test items differed from their spec or the test plan, describe these. Also, if the testing didn't go as planned say WHY, and WHAT was different .
Comprehensive Assessment - How thorough was the testing when compared against what the testplan said it should be? What was not tested well enough, and WHY?
Summary of Results - State here what problems were dealt with, and what probelems still remain.
Evaluation - Don't waffle here, say how good the test items were. Is there a risk they may not be good enough?
Summary of Activities - Outline the main things that happened. What did they cost in terms of people, resources, time and money (you may not know all of these so see the project manager).
Approvals - Get it signed off as a true reflection of the testing efoort and results.
