Memory leak, memory leak will not affect the functionality but it will affect the performance, if you are not using any specific tool to test this then it will not be caught by a black box tester. there could be some other examples also

We can trace the bugs using above said ways, but most of the cases we followed the test case document ie just leave all the cases which are passed, logged the defects which are failed.

Software is never bug free , the high medium and low priority bugs are fixed and some are put in deferred status so that it can be fixed in next version.

As Jitu mentioned no software can be bug free. It just means that all high, medium and some of the low priority and severity bugs have been fixed.

When should I write Bug report?
Is it compulsory or just it helps in the process of testing?
I was thinking about Bug report? Why we should write bug report when we have already bug tracking tool to raise and write down details about it? If we need to write it, where it will be useful?

@gaurav.khurana, shivakumar is right, it need not to be interchanged. And reporting a bug is different from bug report. Bug report is summary of bug reported.
