Template is a general model or guideline that can be use to prepare a document.

Example - For a test plan there would be a template - which can be used by different project to customize or add their content to make it for their project.

A template will show you what to add, a document is actually what is added in template

Template is one which has predefine format for work which the user

dose but the document is one which actual deals which what has been done with the help of template
