When we receive a fix for the raised defect from the technology, we validate that defect from our application and if it is working  as expected then we rerun the script for the defect I.e we do defect retest. To do this your defect should be in retest status in QC. After re running the script we change its status to Retest passed  and assign it to our UAT manager to close the same.

Say, you have found a defect-D1, which is fixed, retested and closed. Now you find another defect-D2 which is fixed and sent for retest. If the D2 fix reopens D1(which is already closed), thats when the closed defect is reopened and tested.

Yes, closed defect has been tested. As a tester, retesting of defect & regression testing further has to be done.
