The main documents needed to write the test cases is SRS. On the basis of SRS the test leads writes the Test Plan . Only after the test plan , the real test cases are written. sometimes USE Cases are also used for writing the test casesThe design documents are not used to write the test cases.

IMHO, Exhaustive testing, testing all the possible test cases (which is in many cases not possible due to time and budget factors)Non-exhaustive testing is executing the chosen test cases based on prirotiy. This is normally followed in many testing projects.
