Within a week of production release after testing 6 months, production enviorment crashed. It was working on Stage/Test envoirnment earlier but later it also stops working. Later reason identified was MS security patch which start blocking COM component. It was fixed and deployed later.

I have recomended Execution sequence plan sheet which contains Requirement Traceabilty Metrics id,Test script id,Resource name,Start date,End date,Execution status,Defect status.by scheduling the process using Execution sequence plan will improve Test process.

I'm not 100% sure if I understood the question, but the verification stage can come as early as during the code review, and can also be done at the Specs and requirements stage (e.g. do the wireframes of the product match what the client asked for?).

While Static Testing, In a FRD, one functionality is not clearly mentioned. As a Tester, you explored the application and identified the flow for that functionality. Now what you will do under this situation?

You report your findings to all internal project stakeholders - BA/ PM etc. You will not get involved with the client directly as there will be internal projects stake holders stationed to speak to the client.

Static testing is review the document before writing test cases. So if any functionality is not clear than document that in the defect report. Because earlier the defect is find out it is less costly.

Phase containment is used for defect prevention so that the defects are found at each phase of the application, an are not multiplied further, we do it by taking JAD sessions, performing reviews after each phase, by participating in walkthroughs, inspection.
