If that is the case then i will file a bug in bug tracking tool then i will approach program manager since it is a tester mistake not the developer mistake.

While testing an application if a defect is found and there is no corresponding test case for the defect, the defect needs to be reported mentioning the severity of the bug. All the steps to be mentioned while reporting a bug.

Cannot predict that it's a Tester Mistake, it can be a CR and which has not been not brought to the Tester and may be the bug is neither relevant to the Requirement nor actual scenario.............many can be the reasons and also first need to analyse the Bug. And not require to take suggestion from the Manager, should be a Trouble shooter

It is a Ad-hoc-testing, means that finding the defects without test-cases. We can directly log this defect in our defect tracker[QC]. If required for long time we can make a test-case for that with Ad-hoc steps and description.

If we dont have a test case, We need to add a Test case for this particular scenario and log a defect in the Bug tracking tool. This will help rectify the issue and now that we have a test case it can be executed again during further test passes or regression phase.

In my project, I have reported such type of bugs through reporting to my project manager at client site.
Defined the severity for the bugs and defined the steps to reproduce the issue which helps for future as well.

Generally, I have done while executing EXPLORATORY TESTING.

Firstly I would make sure that it is a valid defect and has specific requirements attached to the same. If it is valid, then I would make sure that since it does not have a tets case, would talk with the lead and discuss with him how he would like me to approach it,if the defect is a high severity one, definately file it and it could be closed as invalid if needed, the main purpose being here atleast it gor documented.
