after logging in to QC we write testcases in testplan, before writing we select the manul or automated, select QTP. Click on test script we can see icon of QTP,ther u go,click on that icon tool will come up, this integrates QC with QTP, provided u have downloaded the addins for QTP in QC.

When ur working with QTP u can directly save tests, or open defects inQC, by selecting tools and connecting to QC.
