In one of my projects the tests were data driven & the data was so sensitive that erroneous data was throwing errors  which seemed like defects in test environments. Most of time of the BAs & tech leads was being consumed in analysing the seeming defect & coming to conclusion that the root cause was data. So a lesson was learnt - we keenly validated data against FRS as well as got it reviewed once by BAs. since the tests were data driven so the data quantity was astronomical. But the review effort was worth it. Later on UAT ran smoothly with "REAL" defects rather than data-defects.
