This question can be answered in two ways: 

1) If you are performing exploratory testing, the minimum requirements are a product to test and a tester to test it.

2) If you are referring to formal, structured testing, then the minimum requirements depends on the company's entry (or entrance) criteria.  Entry criteria defines the conditions that must be met in order to begin testing.  These are deliverables required by the test team.  They include:

a) All test hardware platforms must be successfully installed configured and functioning properly.

b) All standard software tools including testing tools must be successfully installed and functioning properly.

c) All documentation and design of the architecture must be made available.

d) All personnel involved in the system test effort must be trained in tools to be used during testing process.

e) A separate QA environment (with its own web server, database and application server instance) must be available.

f) Proper test data is available.

g) Software must be unit and integration tested by developers and meet a predefined quality standard.

h) Software build must be staged with proper version control. 

i) Software build must be accompanied by release notes which describe known defects and any other information that would be useful to the test team.
