A) True.  

Edwards Deming advocated that a company should drive out fear.  In his book, "Out of the Crisis" he states, "Drive out fear, so that everyone may work effectively for the company."  In a fearful environment, a tremendous amount of attention and energy is placed on avoiding situations that will produce negative consequences.  As a result, your workforce will be much less likely to admit their mistakes and take risks.
In many companies, the fear factor corresponds to performance reviews, especially in environments where the employee population is placed on a normal distribution and about 10 percent of the workforce will ultimately receive negative reviews (and salary adjustments) based on their poor performance whether they deserve it or not.
